Garra spilota has a restricted range. It is known only from one location in the Rakhine Yoma, in southern Myanmar. Further research is needed to confirm whether this species is only present in this area or is more widespread. Although no important threats have been identified in this area its restricted range makes the species vulnerable to any future threat. The species is therefore listed as Near Threatened.
